Adoption Application
The first step in adopting a bird through HFFN. Covers your living situation, bird experience, household, and what you are looking for in a companion. Approval is not guaranteed and all applicants are reviewed individually.

For members interested in providing temporary foster care for birds in HFFN’s network. Covers your home environment, existing flock, quarantine capability, and availability. Foster families are essential to how HFFN operates.

DownloadSurrender & Bird Intake Forms
Forms for owners surrendering a bird to HFFN and for documenting birds entering our network. Please read the Rescue, Rehoming & Surrender page before submitting — all surrenders are reviewed individually and HFFN cannot guarantee immediate placement.
Surrender Form
For owners who need to find a new home for a bird. Covers the bird’s species, age, health history, temperament, diet, and the owner’s timeline and reason for surrender. Submission begins a conversation — it does not constitute an automatic acceptance.
DownloadBird Intake Form
Completed by HFFN at the time a bird enters our network — whether through surrender, confiscation, or emergency placement. Documents the bird’s physical condition, known history, banding or microchip information, and any immediate health concerns.
DownloadMedical Authorization & Veterinary Forms
Forms governing veterinary care for birds in HFFN’s care. HFFN’s primary veterinary partner is Dr. Walsh at Feather and Fur Animal Hospital in Kailua.
Bird Medical Authorization
Authorizes HFFN or a designated foster family to approve necessary veterinary treatment for a bird in HFFN’s care, up to a specified dollar threshold. Signed by the surrendering owner at the time of intake or by HFFN for birds entering without a known prior owner.
DownloadVet Referral Letter
A template letter from HFFN referring a bird in our care to a veterinary provider, confirming the bird’s status in our network and authorizing examination and treatment. Board members complete and print this on a per-bird basis as needed.

HFFN’s official policy governing volunteer engagement: roles and responsibilities, conduct standards, confidentiality of member and bird records, media policy, and the organization’s commitments to its volunteers. Read before signing the agreement.

Home Inspection Checklist
The standard checklist completed by an HFFN inspector during an in-person home visit. Evaluates safety hazards, cage space and placement, ventilation, toxic plants or materials, other animals in the home, and compliance with HFFN’s housing standards.

Donation forms, receipts, and membership payment options. HFFN is a 501(c)(3) nonprofit — charitable donations are tax-deductible to the extent permitted by law. You can also donate directly at Support HFFN.
Donate for Membership
A combined form for community members who wish to make a charitable donation in lieu of or alongside a standard membership fee — including supporters who are not local to Oʻahu but wish to contribute to HFFN’s mission. Documents donation amount, donor information, and desired membership designation.
DownloadDonation Receipt
An official HFFN donation acknowledgment letter for tax purposes, documenting the donor’s name, donation date, amount, and confirmation that no goods or services were provided in exchange. Required by the IRS for deductions of $250 or more. Issued by HFFN upon request.

Photo & Media Release
An individual signed release for members, attendees, or volunteers who wish to provide explicit written consent for HFFN to use their likeness and/or their bird’s image in HFFN publications, social media, and organizational materials. Optional — event attendance carries implied consent per HFFN’s Photo & Media Policy.
